Visits to the Waʻa

Submit Completed waiver forms to KVA - due 1 week in advance

  1. Complete Waiver Forms:  Please have one waiver form completed for each individual that will be boarding the wa'a.  A copy of the waiver form can be downloaded below.   
  2. Submit Completed Forms:  Scan completed forms and email to [email protected].  They can also be given to KVA staff during class visits, or they can be mailed or dropped off at:

Visits to Kualoa Regional Park

Apply for park permits and/or indemnification

  1. Apply for Park Permit:  All schools and organizations visiting Kualoa Regional Park must apply for a park permit.  The application must be submitted at least 45 days in advance of your visit.
  2. Apply for Indemnification (DOE, UH and charter schools only):  See Indemnification Application Schedule below.
